* . This product covers medical expenses for hospitalization in all public hospitals throughout the country and private hospitals approved by FWD. For private hospitals, the customer does not need to medical expenses first i.e.either both in-patient and out-patient. (you pay only the excess medical expenses.) In case of illness, the policy becomes valid 30 days after the policy issue date. In case of accident, it is immediately in force. FWD Life Insurance Public Company Limited is the only company which offers there benefits.
* For public hospital, we will reimburse you based on your receipts, but not exceeding coverage limits.

Conditions:
Issued Ages: at last birthday 1 month 1 day – 60 years (expand to 69 years).
Must have base plan first except Term(Non-par) (T15, T10, UE10 and UE25/7).
When total current premium is over 50,000 baht, the maximum room and meal rate, board of all PPO 5,000 baht,
When total current premium is over 100,000 baht, the maximum room and meal rate, board of all PPO 10,000 baht,
Yearly or a half yearly, quarter yearly or monthly payment
PPO3000 and PPO5000, the PPOSOS 1 will be added.
PPO800, PPO1400 and PPO2000, the PPOSOS 2 will be added.

   The Level of service depends on your PPO code, shown on your care crad.

 

A. International Assistance for Travel and Medical
The user shall be eligible for services when he/she travels outside Thailand for periods not exceeding 90 consecutive days per trip.
 
1) International Travel Assistance
  1.1 Pre-trip Information Services: International SOS will provide information concerning visas and inoculation requirements for foreign countries worldwide including information service of Temperature for all countries and foreign Exchange rate.
1.2 Embassy Referral: International SOS shall provide the address, telephone number and hours of opening of the nearest appropriate consulate and embassy worldwide.
1.3 Lost Luggage Assistance: International SOS will assist the insured who has lost his/her luggage while traveling outside Thailand by referring the insured to the appropriate authorities involved.
1.4 Lost Passport Assistance: International SOS will assist the insured who has lost his/her passport while traveling outside Thailand by referring the User to the appropriate authorities involved.
1.5 Emergency Message Transmission: In the event of a medical emergency, International SOS will assist the User to transmit urgent messages to family members, friends or business associates upon the User’s request.
1.6 Interpreter Referral: International SOS will provide the names, telephone numbers and, if possible and requested, hours of opening of interpreters‘ office in foreign countries by telephone.
1.7 Legal information : Provide name , address, telephone number and business hour of lawyer and legal practitioners worldwide
  2) International Medical Assistance
  2.1 Telephone Medical Advice: International SOS will arrange for the provision of medical advice to the insured over the telephone.
2.2 Medical Service Provider Referral: International SOS shall provide to the insured, upon request, the name, address, telephone number and, if available, office hours of physicians, hospitals, clinics, dentists and dental clinics (collectively, „Medical Service Providers“). International SOS shall not be responsible for providing medical diagnosis or treatment.
2.3 Arrangement of Appointments with Local Doctors for Treatment: International SOS will assist the insured by arranging for appointments with local doctors for treatment.
2.4 Arrangement of Hospital Admission: If the medical condition of the insured is of such gravity as to require hospitalization, International SOS will assist such insured in the hospital admission.
2.5 Guarantee of Medical Expenses Incurred during Hospitalization: International SOS will assist the User by guaranteeing on behalf of the User medical expenses incurred during his/her hospitalization when get approval from ING
2.6 Arrangement of Emergency Medical Evacuation for insured: International SOS will arrange for the provision of air and/or surface transportation, medical care during transportation, communications and all usual ancillary services required to move the User when in a Serious Medical Condition to the nearest hospital where appropriate medical care is available. International SOS will arrange for the provision of appropriate communication and linguistic capabilities, mobile medical equipment and medical escort crew.
2.7 Arrangement of Emergency Medical Repatriation for insured : International SOS will arrange for the return of the User to Thailand following the User’s Emergency Medical Evacuation and subsequent hospitalization outside Thailand. International SOS will arrange for the provision of appropriate communication and linguistic capabilities, mobile medical equipment and medical escort crew.
2.8 Arrangement of Transportation of Mortal Remains: International SOS will arrange for the transportation of the insured’s mortal remains to Thailand or arrange, as requested by the User’s family, for local burial at the place of death. This is under SOS evaluation.
Note: Service at item 6 to 8 are on maximum service condition.
   
B. Domestic Medical Assistance
The Users shall be eligible for Services when they travel more than 150km from the place of residence and within Thailand for periods not exceeding 90 consecutive days per trip.
  1) Medical Service Provider Referral: International SOS will provide the name, address, telephone number and, if requested by the User and if available, office hours for physicians, hospitals, clinics, dentists and dental clinics (collectively, „Medical Service Provider“).
2) Arrangement of Hospital Admission for insured : If the medical condition of the insured is of such gravity as to require hospitalization, International SOS will assist the User with hospital admission.
3) Arrangement of Emergency Medical Evacuation for insured : International SOS will arrange for the air and/or surface transportation and communication for moving the User when in a Serious Medical Condition to the nearest hospital in Thailand where appropriate medical care is available.
4) Arrangement of Emergency Medical Repatriation for insured : International SOS will arrange for the return of the User to the place of residence in Thailand following an emergency medical evacuation for subsequent in-hospital treatment more than 150km from the place of residence and within Thailand.
5) Arrangement of Transportation of Mortal Remains: International SOS will arrange for transporting the insured’s mortal remains from the place of death to the place of residence.
Note: Service at item 3 to 5 are on maximum service condition.
